RAADS-R scoring and what scores mean

Short answer
RAADS-R scoring gives each of the 80 statements 0 to 3 points, so totals run from 0 to 240. The 64 symptom statements score highest for "true now and when I was young", and 16 statements are reverse scored. The authors reported that a total of 65 or more was consistent with a clinical diagnosis in their study. A score supports a clinical conversation; it is not a diagnosis.
People usually look up RAADS-R scoring after they already have a number. This page explains where that number comes from, what the published threshold was based on, and why the many online "score charts" with labeled bands do not come from the authors.
How each answer is scored
Every statement has four answer options about timing. For the 64 statements that describe autistic experiences, "true now and when I was young" scores 3, "true only now" scores 2, "true only when I was younger than 16" scores 1, and "never true" scores 0. The other 16 statements describe common, non-autistic experiences and are flipped, so "never true" scores 3 on those. Adding all 80 gives the total.
| Answer | Symptom statements (64) | Reverse statements (16) |
|---|---|---|
| True now and when I was young | 3 | 0 |
| True only now | 2 | 1 |
| True only when I was younger than 16 | 1 | 2 |
| Never true | 0 | 3 |
Maximum score and score range
With 80 statements worth up to 3 points each, the highest possible total is 240 and the lowest is 0. Nobody in the validation study reached 240. The highest autistic score reported was 227, and the lowest was 44.
The cut-off of 65
The authors used statistical analysis to find the total that best separated autistic and non-autistic participants in their sample. That point was 65. Their article states that a RAADS-R score of 65 or greater is consistent with a clinical diagnosis of autism. Notice the wording: consistent with, in a sample of people whose diagnoses were already confirmed. It does not mean that 65 or more proves autism or that 64 rules it out.
Average scores in the original study
There is no official average for the general population. The validation paper reports group averages for its own participants, which gives a rough sense of scale but should not be read as norms.
| Group | Number | Mean total |
|---|---|---|
| Autistic disorder (DSM-IV) | 66 | 138.46 |
| Asperger's disorder (DSM-IV) | 135 | 131.53 |
| No psychiatric diagnosis | 276 | 21.39 |
| Other psychiatric diagnosis | 302 | 30.11 |
About online score charts
Many websites show charts that label totals as low, moderate or high, or attach phrases to ranges like 90 to 130. Those bands are not in the 2011 paper. The authors published one threshold and the group results above. When a chart claims more than that, it is someone else's interpretation, and you can treat it with caution.
Subscale scores
The 80 statements also split into four areas: social relatedness, circumscribed interests, language and sensory motor. Clinicians sometimes look at these to see where a person's answers cluster, for example many sensory statements but few language ones. The paper analyzed each subscale and found the total score separated groups most accurately, so subscales are best read as a description of where your answers fall rather than as separate tests.
That overlap is not a flaw unique to the RAADS-R. Any self-report questionnaire asks you to judge your own social life, sensory experiences and childhood, and those judgments are shaped by mood, stress, and how you compare yourself with others. Someone with social anxiety may strongly endorse statements about finding conversations hard for very different reasons than an autistic person would. This is why a total is best treated as a summary of how you described yourself on one day.
Reading your own score sensibly
- A score of 65 or more means your answers resemble those of autistic adults in the validation sample. It is a reason to consider a professional assessment if you want one.
- A score under 65 does not rule autism out, especially if you mask, were unsure about childhood answers, or took a copy on your own.
- Anxiety, ADHD, trauma and low mood can all raise scores on several statements.
- A very high score does not measure how autistic someone is or how much support they need.
Frequently asked questions
What is the highest possible RAADS-R score?
The maximum is 240, because each of the 80 statements is worth up to 3 points. In the 2011 validation study, the highest score among autistic participants was 227. Very high totals do not indicate a stronger diagnosis or greater support needs.
What does a RAADS-R score of 65 mean?
The authors found 65 was the point that best separated autistic and non-autistic participants in their study, and they described 65 or more as consistent with a clinical diagnosis. It is a reason to discuss an assessment, not a result in itself. Only a qualified professional can diagnose autism.
Can you score high on the RAADS-R and not be autistic?
Yes. Studies in clinic samples found many people without autism, especially those with other mental health conditions, scored above the threshold. A French study reported a false positive rate of 55.6% in its psychiatric comparison group. Context and a full assessment matter.
Is there an official RAADS-R score chart?
The official scoring sheet tells clinicians how to add points and gives the threshold of 65. The labeled bands found on many websites are not part of the published scale.
